Student Health Portal & Health Forms
The student health portal is a one-stop place for students to easily submit health forms, send secure messages to the health & counseling staff, and quickly access resources to help support mental and physical health. Students are required to complete several health forms, including one that needs to be printed and taken with them to their healthcare provider to complete and sign. All forms can be found in the student health portal.
Student Health Portal Physical Form for Student Athletes Medical Release of Information
For medical needs, students are asked to call ahead to (315) 279-5368 to schedule an appointment with our healthcare providers. We bill insurance for medical appointments, so students will need to provide updated health insurance information at the time of visit.
Students are encouraged to be familiar with their health plan, particularly if coverage is "out of network" or if pre-authorization is required for visits to the Health Center.
Our healthcare providers can call in prescriptions to area pharmacies. We work with pharmacies in Penn Yan that can deliver prescriptions or over-the-counter medication to campus free of charge.

Are you feeling stressed, alone, anxious, overwhelmed, down, or different?
The Health & Counseling Center offers individual therapy in-person or via teletherapy for comfort and convenience. Counseling services are available to students enrolled at Keuka College for no cost.
We use a brief solution-focused approach to assist with concerns such as anxiety, depression, grief and loss, stress management, adjustment to life challenges, and relationship issues. Students who need ongoing counseling, or counseling that is deemed beyond the scope of practice of the Center, will be referred by Counseling Center staff to local resources or resources closer to the student’s home.

Keuka College is excited to offer students access to the OASIS app, a 24/7 support service designed to help manage common life stressors and enhance well-being. Funded through our SAMHSA Mental Health Awareness Training (MHAT) grant, OASIS provides a variety of self-help resources and guided chat support to empower students to handle stress, build resilience, and maintain their mental health.
The app offers solution-based motivational interviewing through a confidential chat function, monitored by mental health professionals to ensure student safety. If a student experiences more serious concerns, the app’s monitoring system will notify the College so we can provide immediate support.
OASIS is part of our broader commitment to proactive wellness and upstream prevention – focusing on preventing challenges before they become severe. This aligns with our three-tiered approach to student well-being and the eight dimensions of wellness that are at the heart of our co-curricular programming in both Academic Affairs and Student Development.
Download the OASIS app to find tools and support tailored to your needs and well-being!
